I hereby agree that Fairplay Sports coaches and employees are released from any liability and responsibility for injury, illness, death and/or loss of property whatsoever which may occur to my child (or child in my care) at any time during group tennis sessions and I agree to indemnify Fairplay Sports coaches and staff from and against all liability damages and claims whatsoever in respect thereof. I understand that the tennis session involves physical activities and I can confirm that my child (or child in my care) is able to safely participate in those activities.Should my child be injured during the tennis session, I give Fairplay Sports coaches permission to call an ambulance for transport to local hospital.I have listed above any allergies that my child has to either food, medicine or first aid items
